COVID-19 Grant Certification Form 5734

    Dear Staff Member,

    On Monday, November 2nd, the Michigan Department of Treasury released guidance on Teacher and Support Staff COVID-19 grants.  This information is in reference to legislation that recognizes the extenuating circumstances that our school employees were in during the end of the 2019-2020 school year.

    These grants are awarded in the amount of up to $500 to eligible teachers that worked during the 19-20 school year.  Support staff or non-instructional personnel are eligible for up to $250.

    To be eligible as a teacher for these grant funds, you must certify three affirmations of the grant eligibility requirements.

    1.  That you performed 75% of your workload in a brick/mortar school building.  
    2. During the time of April 2nd through the end of the school year, you developed tools and methods to deliver distance learning, take-home packets, or other methods described in the district’s continuity of learning plan. 
    3. You will have to indicate that you did one of the following during this time frame:  Worked additional time spent outside of normal working hours, experienced hazardous conditions, or incurred additional costs related to ensuring students could effectively participate in the school’s continuity of learning plan. 

    To be eligible as a Support Staff or Non-Instructional Personnel for these grant funds, you must certify two affirmations of the grant eligibility requirements.

    1. That you performed 75% of your workload in a brick/mortar school building.  
    1. You will have to indicate that you did one of the following from April 2, 2020 until the end of the school year:  Worked additional time spent outside of normal working hours, experienced hazardous conditions, or incurred additional costs related to ensuring students could effectively participate in the school’s continuity of learning plan. 

    The required form from the Michigan Department of Treasury can be found below. Download the form and perform the following steps to apply for this COVID-19 grant award:

    1. Complete Form 5734 in its entirety.  Please print it, sign it and return the paper copy to the Personnel Office prior to 5 p.m. on Friday, December 4th.  If you cannot find your PIC number, please leave that category blank and we will fill it in for you.  Please note that December 4th is a hard deadline as Treasury has made it very clear that forms turned in late will not be accepted.  You can inner-office mail it to me or bring it over to the Administration Building.  To help safeguard sensitive information on the form, please do not email or scan any forms back to me.    
    2. After December 4th, the Personnel Office will review all submitted forms and will submit roster information to the State Treasury as required.
    3. Checks to eligible employees will be mailed by February 25, 2021 to the employee’s residence directly from the Michigan Department of Treasury.  Please make sure your address is correct and fully completed on Form 5734.
